For a Birth Certificate
This is the first official identity document for a child, essential for school admissions, passports, and more.
- Proof of Birth: A letter or certificate issued by the hospital or nursing home where the child was born.
- Parents’ Identity Proof: A copy of the Aadhaar Card or Voter ID of both the mother and father.
- Parents’ Address Proof: A copy of a recent utility bill, passport, or bank passbook.
- Parents’ Marriage Certificate: A copy of the marriage certificate is often required to establish the parents’ names.
- Application Form: The fully filled registration form provided by the municipal office or hospital.
- Affidavit (for delayed registration): If the birth is not registered within 21 days, an affidavit from a notary explaining the reason for the delay is usually required.
For a Death Certificate
This document is legally required to settle inheritance, claim insurance, and for various other official purposes.
- Proof of Death: A certificate from the doctor who declared the death or a report from the hospital.
- Deceased’s Identity and Age Proof: A copy of the deceased’s Aadhaar Card, PAN card, or Birth Certificate.
- Deceased’s Address Proof: A copy of a utility bill or ration card for the deceased’s last address.
- Applicant’s Identity Proof: The Aadhaar card of the person applying for the certificate (usually a close relative).
- Cremation/Burial Receipt: The receipt from the crematorium or burial ground.
- Affidavit (if required): An affidavit specifying the date, time, and cause of death, especially if the death occurred at home.
